Normal:
01:29:65 (3,1) - this threw me off because it is the only set of beats in this diff that have a half note difference

Hard:
nothing

Insane:
The beginning seems to harder than the rest of your song
00:19:55 (2,3) - can you please not put a stack in a deathstream

I didn't quite fully comprehend on what you said here so I tried my best. Hope it's right now.

Try -5ms to all timing section (Vocal is a bit early than drum beats)
you can test it if you don`t feel it`s better
just keep your old offset
