Offset 562 is better.

If you change it (and better if you change it) you must fix all time sections.
Don't put time sections on notes if you not sure about offset.

I don't think gray is good color with this bg. Hard read streams on insane, cause better if you change it.
Ahh.. also, gray + Kiai time = Wtf!? You must change it.

4.5 stars for normal is.. uh.. it's fine for me, but i'm not sure about BATs.

In some moments i don't understand rhythm, like:

00:18:480 (2,3)
00:30:567 (1,2)

You can put one bit on 00:19:128 and remove slider, it's looks better, right?

Make lead-in 1.5 or 2 seconds

Easy:
00:53:33 timing section isn't needed
I would use the largest circles for this map
01:19:991 (1) - idk how BAT's will react to this, but it seems ok to me

Normal+:
Alot of people don't like the + after the difficulty name, you might to just rename this fnia's Hard
Make sure to move all timing sections that effect the audio (or change it to soft etc) 1/4 before the note you want it to effect
End kiai time somewhere near the end as all you need is the fountain from it

Hard:
00:53:33 timing section isn't needed
map looks fine

Insane:
