00:12:78 (5) - Spaced confusingly close, I think. I know 00:02:78 (5) is spaced even closer relative to time, but the fact that it's so far away on the timeline there actually works against people being thrown off.
00:31:78 (5,6) - From the previous stanza, the player knows that the singer will sing two syllables at that point, the second falling on the half beat. Combined with the fact that these are stacked circles, I would always click the 6 sooner. Unless you consciously intend to trick players, either unstack the two circles or move the 6 1/2 backwards.
00:51:45 (6) - Make the end fall on the preceding white tick.
00:53:61 (4) - Same.
01:10:78 (5) - Doesn't feel mapped to anything in particular. Move 1/2 forward?
01:13:11 (2) - Space it farther as in the first item on my list.
There are some other spacing inconsistencies, but like I said before I don't think they are confusing enough to merit change.

I also wonder if the titles do not convey each difficulty level well enough to anyone who doesn't know of the show; although, not knowing it, I am of course biased.
