Nope, this isn't lag related. The HP will suddenly just skip to completely full instead of slowly filling up during lead-in time and SURPRISE, there's also a hit circle you have to hit at this exact moment.

OH WHAT ROFL. If I turn on auto while I'm bugged out like this, it will instantly play like 1/3rd of the song and miss several times.

I had this happen several times but couldn't figure out how to reproduce. Also, it happened once a month or so so a restart didn't bother me too much.

Using your steps, I wasn't able to reproduce, though.
