Timing: Pretty solid to me.

Medium: (Sure you don't want to use Normal?)

00:51:74 (1) - Place this farther from 2. Distance snap accordingly.
02:16:56 (1,2,3) - Space these out more so they are as spaced as the green beats after it.
03:03:98 (1) - I'd delete this beat and just have it end at the spinner.

Hard:

00:59:37 (1,1) - Shift the red 1 1/4 beat forward and decrease length by 1/4, Add 1 repeat on the blue 1.
01:58:45 (1) - I dunno about a jump that big on hard (from the green 1). Kinda pushing it, but I can let it slide.
03:03:98 (1) - See Medium.

Insane:

The 1/8 note spam during the chorus is definitely insane. <_<

Otherwise, nothing noticeable.

You get a star.

insane:
00:35:54 - 2 and 3 should be snapped at the blue and red tick respectively. sounds the same to me. same as the blue and red slider 1.
