Timing needs work. I found that a 135 BPM and 21,987 Offset works well.

Mapping is pretty good, you just need to work on cleaning up your spacing a bit. Seeing as how this is Insane, some of it can be kept in. I can give you some suggestions once you fix the timing and resnap/recalc your notes.
